DISCHARGE PLANNING

If you or a loved one has found themselves hospitalized or is currently admitted, it can be overwhelming. We are here to provide solutions when discharge home is no longer the safest option.​  ​

This is often the case with seniors who are experiencing a decline in cognition as seen with dementia and Alzheimer's. It becomes even more challenging when behaviors are involved.

 

As a result, we work closely with families and case managers to create safe discharge plans. We have a network of assisted living homes and mobile providers who have expertise in de-escalation, redirection and decreasing stimuli. 

After discharge we continue to follow our clients, which has been significant in reducing the risk for hospital  readmission.
